Job Summary

The Community Registered Nurse plays a central role in ensuring that families and clients receive the highest level of care. The Community Nurse provides in-home client intake assessment and works closely with our care coordinators and clinical care managers to develop detailed and customized care plans specific to our clients. The Community Nurse is guaranteed one to seven day’s worth of assessments per week in the borough of their preference, depending on their availability. If assessments are not required during the guaranteed day(s), the Community Nurse will have the opportunity to work in our corporate office, assisting with caregiver orientations, providing further education to care providers, and participate in both follow up and quality assurance during in-home client visits.

Compensation and Benefits

  • Start with a guaranteed 1 to 7 days per week as Community Nurse. 
  • Continue to work with Mavencare on per-diem basis for other days.
  • Opportunities to grow into other roles as the organization expands- such as Nurse Manager, Home Care Administrator, or Community Director, Director of Patient Services.


  • Being accountable for assessments scheduled on the agreed day(s) of the week in the boroughs / counties assigned to you.
  • Perform in-home assessments and create Plans of Care for new patients and complete follow-up assessments for existing patients.
  • Complete and submit assessment paperwork in a timely and organized manner.
  • Assist the Director of Patient Services in assessing the competency and performance of Home Health Aides.


  • Graduate from an accredited School of Nursing, BSN or BS degree preferred.
  • New York State licensed and currently registered as a Registered Nurse.
  • Graduate from an accredited School of Nursing, BSN or BS degree preferred.
  • At least three years of experience in a patient care setting. Acute care experience desirable.
  • At least three years of experience in a supervisory or administrative position in a home care organization or equivalent.
  • Strong working knowledge of NY State regulations as they relate to the operation of a Licensed Home Care Service Agency.
  • Expertise and experience in dementia care is highly desirable.
  • Ability to work effectively to address challenges as part of an interdisciplinary team.
  • The following competencies are also required:
  • Competence with Microsoft Office suite and Google Docs.
  • Comfort working with and managing teams using process and policy documents.
  • Comfort presenting to groups of clients or employees of the agency.
  • Ability to write concise reports.
